Abstract

Recently, in the era of the fourth industrial revolution, the main technology trend of digital dentistry is to make dental prostheses using 3D printer. In this study, we evaluated the accuracy of dental prostheses(coping) made with metal 3D printer. The equipment used in the experiment are Cr-Co 3D printer, 3D scanner, Analysis Software(Geomagic). and Experimental process parameters are hatching(1way, cross, counterclockwise) and orientation(0 degrees, 45 degrees, 90 degrees). The 3D printing accuracy of the dental prostheses(coping) was excellent for “Hatching cross” and “Orientation 90 degrees.” That is, The dental prostheses(coping) manufactured by using 3D printer has higher accuracy with higher heat emission. If the angle of orientation is less than 45 degree, the roughness of the lower part of the coping is not particularly good. and this has a negative effect on the suitability of inner side crown.

1. 서 론

3D 프린팅이란 3D 스캐너 기기로 물체를 인식한 후 3D 모델링 소프트웨어로 그린 설계도를 이용하여 잉크젯 프리터와 같은 원리로 금속이나 플라스틱 같은 물질을 분사하거나 적층하는 방식으로 층층이 쌓아 올려 입체적인 물건을 출력하는 기술이다. 이와 같은 3D 프린팅 기술은 미세한 물질을 쌓아 올려서 물체를 조형 한다고 해서 적층가공(Additive manufacturing)이라 부르기도 하며 넓은 의미의 쾌속조형(Rapid prototyping, RP)의 한 기술로 분류되기도 한다. 치과 3D 프린팅 기술이 미래의 주요 기술로 자리매김하고 있고 신성장 동력원으로 제조업, 의료업계 등 국가 산업 전반에 걸쳐 새로운 동력을 제공할 수 있는 핵심 기술로 급부상하고 있다1,2).

일반적으로 보철물은 치과기공사의 수작업에 의해서 제작되었지만, 최근 들어 정밀성/생산성/단납기 등의 요구로 인하여 CAD/CAM 기술을 활용한 형태로 변화하고 있다. 이를 반영하듯, 미국, 유럽 등에서는 비접촉식 스캐너(레이저, 혹은 광학식 스캐너)를 이용하여 치아 형상 데이터를 취득하고, 인공 보철물을 가공하는 치과용 CAD/CAM 시스템의 시장이 점차 증가하고 있다. 치과용 CAD/CAM이란 치과 진료 과정에서 필요한 장치물들(예: 보철물, 수복물, 교정장치, 임플란트 시술용 Stent)을 컴퓨터로 3차원 스캔하여 컴퓨터상에서 치아 보철물을 직접 디자인(CAD)하고 이를 기계로 정밀하게 절삭(CAM)하여 제작하는 것이다. CAD/CAM의 도입으로 인해 우수한 기계적 성질과 생체 친화성을 가지고 있는 재료들을 이용한 장치물의 제작이 용이해졌다3-5).

본 연구에서는 CAD로 보철물을 디자인한 후 도포된 금속분말에 선택적으로 레이저를 조사하여 용융시키고 한 층씩 쌓아올려 복잡한 형상의 의료용 임플란트 제작에 적합한 적층방식인 SLM(Selective Laser Melting) 기술6)(Fig. 1)을 이용하여 제조방향(Orientation) 및 적층방향(Hatching) 조건에 따라 금속 보철물의 정확성을 확인하고, 금속 3D 프린팅 기술의 디지털 치과 산업(Digital dentistry) 활용 가능성에 대해 알아보고자 한다.

2. 실험 방법

2.1 보철물 제작 및 스캔

치과 임상에서 사용하고 있는 상악 우측 중절치를 실험에 사용할 지대치로 선정하고 치과용EXO 캐드 소프트웨어를 사용하여 실험 데이터를 설계하였다(Fig. 2).

추출된 코핑 3D 모델링 데이터를 이용하여 금속 3D 프린팅을 실시하였고, 정확한 변형량 및 내외면 오차율을 측정하기 위해 적용된 서포트를 제외한 나머지 부분은 후가공을 처리하지 않았다. 스캔장비는 Amann Girrbach사 MAP400 3D 스캐너(Scan accuracy: < 20μm, Scan speed: Single Die 30초-unit Bridge Die 100초 이하)를 이용하였다.

스캔방법은 코핑 내외면의 변화량을 확인하기 위해 코핑 Tip부분을 Blue tech(접착제)으로 고정시킨 후 약 20초 간 광학(Optical) 방식의 치과용 3D스캐너를 이용하여 스캔을 진행하였다. 금속의 경우 난반사로 인해 스캔이 어렵기 때문에 스캔 전용 스프레이를 약 10~ 20μm 두께로 도포 후 측정하였다.(Fig. 3)

1차 기본 스캔 후 각 영역을 회전시키며, 추가 스캔을 약 5회 진행하였으며, 제작된 코핑 시편 9개 모두 동일한 방식으로 3D 스캔하였다.(Fig. 4) 오토 Fixing 기능이 자동 적용되어 Fill Hole 및 Surface Smoothing 작업을 진행하였고, 크라운 외 불필요 영역(Blue Tech, Jig 등) 삭제 후 최종 완료된 STL 데이터를 추출하였다.(Fig. 5)

2.2 금속 보철물 제작

스캔한 상악 우축 중절치 11번 코핑 STL 파일과 Cr-Co 3D printer system(SLM125, Realizer, Germany, Fig. 6)을 이용하여 제조방향(Orientation)과 적층방향(Hatching)에 따라 조건을 달리하여 제작하였다. (Table 1)

공정변수는 Orientation 0°, 45°, 90°(Fig. 7), Hatching 조건 1way(X축으로만 적층), 2way(90°, 180° 가로, 세로 번갈아 가며 cross 적층), 2way(45° 시계반대방향으로 돌아가며 적층)이며(Fig. 8), 기타 레이저 파워(200W), 적층두께(0.025mm), Hatching Distance (0.08mm), 사용가스 Argon, Hatching Offset (0.04mm), Laser Exposure Time(60μs), Point Distance(20μm)는 동일한 상태에서 측정하였다.

2.3 전용 소프트웨어를 이용하여 분석

스캔된 STL 파일을 총 3개의 군(A군, B군, C군)으로 나누고 대조군(원본스캔파일), 실험군(A군, B군, C군)을 3차원 중첩 분석 프로그램(Geomagic Control X, 3D Systems, USA)을 이용하여 중첩, 분석을 시행하였다. 파일의 크기를 줄이고 용이한 분석을 위해 디지털 모형을 변연 하방으로 약 0.2~0.3mm 정도 남기고 자른 뒤 전체적인 오차가 최소가 되도록 하는 최적중첩(best-fit algorithm) 방법을 사용하여 디지털 모형을 중첩시켰다. 색지도 측정법과 히스토그램 분석을 이용하여 대조군과 실험군 사이의 3차원적 오차값이 얻어지고 계산되었다.

3. 실험 결과 및 고찰

3.1 서포트 제거 전후에 따른 출력물 비교

적층방향(Hatching)과 제조방향(Orientation) 조건에 따라 각각 3D 프린팅 스캔파일을 이용하여 치과 임상 보철물을 제작하였다. 서포트 제거 전후의 3D 프린팅 출력된 결과물은 아래 Fig. 9와 같다.

3.2 원본 스캔파일과 3D 프린터 출력물 스캔파일 값 비교 분석

원본 스캔파일과 3D 프린터 출력물 스캔파일을 3차원 이미지 분석으로 중첩시킨 이미지 결과물은 Fig. 10과 같다.

히스토그램을 분석하였을 때, A군 표준편차는 90°: 0.074mm, 45°: 0.107mm, 0°: 0.149mm가 나타났으며, B군 표준편차는 90°: 0.066mm, 45°: 0.142mm, 0°: 0.079mm가 나타났으며, C군 표준편차는 90°: 0.099mm, 45°: 0.147mm, 0°: 0.105mm가 나타났다(Table 2).

A군의 적층 방법(Hatching)이 가장 편차가 심한 이유는 Laser를 사용하여 파우더 Melting 시 발생되는 Melt pool의 경우 동일한 방향으로 계속 진행되면 Cooling 되는 과정에서 두께 변화량이 발생될 수 있기 때문에 Porosity가 생성되어 정밀도에 영향을 미칠 수 있으나, B군과 C군은 Hatching 방향을 2Way로 설정하여 Melting면의 Cooling control이 가능하고, 리코팅에 의한 형상 무너짐이나 계단식 적층에 있어 형상 변화량을 최소화 할 수 있기에 상대적으로 A군 보다 우수한 정밀도 결과를 얻을 수 있었다. 특히 B군의 Cross hatching은 C군의 45° 시계반대방향 Hatching 보다 냉각속도에 따른 열방출이 크기 때문에 더 낮은 내부 Porosity 영향으로 보다 양호한 정밀도를 얻었다고 판단된다7).

또한 제조방향(Orientation)의 각도가 45° 이하일 경우 출력물의 처짐 현상이 발생될 수 있고 하단부 표면조도가 매우 나쁘게 나오기 때문에 Crown 내면의 적합에 영향을 미칠 수 있다. 따라서 출력물 제조 방향은 각도가 90°일 경우 치과 임상 보철물의 내면 적합도가 가장 우수하다고 판단된다.

4. 결 론

본 연구는 동일한 모델 스캐너를 이용하여 원본 스캔파일과 3D 프린터 출력물을 스캔하여 얻은 보철물을 3차원적으로 중첩시켜 비교하여 다음과 같은 결론을 얻었다.

1) 적층방향(Hatching) cross scanning으로 3D 프린팅 한 B군이 더 적은 표준편차를 보였으며, 이는 적층방향에 따른 공정변수가 레이저 열원의 에너지 밀도 외에도 냉각속도와 같은 열방출 효과로 인하여 치과용 보철물 코핑 형상의 밀도 변화 및 정밀도에도 직접적인 영향을 주는 것으로 판단된다.

2) 제조방향(Orientation) 적층 조건을 90°로 제작한 출력물이 치과 보철물의 내면 적합도가 우수한 양상을 보였다. 즉, 각도가 낮으면 낮을수록 정밀도가 떨어지며 중력에 의한 표면 쳐짐 현상 발생으로 전체 형상이 변형될 수 있다.
